Multi-physics configuration

Hallo,
I am relatively new in comsol and facing a problem to solve a coupled problem. I am trying to solve a couple (heat transfer and diffusion) problem but i don't know how to configure it. I need to solve concentration changes due to diffusion which is Temperature dependent. So i'm taking Temperature from first solution and want to calculate D (diffusion cofficient), then want to run the second simulation. But i don't know how to configure it, i mean where to change the configuration so that i can take Temperature from 1st solution and calculate D (diffusion cofficient). In my project (D=c). I'm attaching the project with this message, so anyone can take a look and give me a solution.

I would be so grateful to have any kind of suggestion.
